Marcus was the son of Marcus III Livius Drusus and Servilia of Rome. The date and place of his birth have not been found.
His wife was Alfidia of Rome. They were married, but the date and place have not been found. Their only known child was deified Livia Drusilla (0058BC-0029).

According to Suetonius, Marcus was a direct descendant of the consul and censor Appius Claudius Caecus. He was descended from Caecus via from the first Appius Claudius Pulcher, who was consul in 212 BC and Caecus's great-grandson.
